Surgical Treatment of Adult Extra-Articular Distal Humeral Diaphyseal Fractures Using an Oblique Metaphyseal Locking Compression Plate via a Posterior Approach

چکیده
Objective: The purpose of this study was to evaluate the feasibility and clinical outcome of using an oblique AO (Association for the Study of Internal Fixation) metaphyseal locking compression plate (MLCP) in the treatment of adult extra-articular distal humeral diaphyseal fractures via a posterior approach. Subjects and Methods: A series of 19 cases of adult extra-articular distal humeral diaphyseal fractures were surgically treated using an MLCP and lag screws, if necessary, via a posterior approach. All patients were followed for a mean time of 16.6 months (range from 12 to 20 months). The patients were followed up to 1 year, and radiological assessment was performed to observe fracture reduction and healing. Functional outcome was measured including the range of motion of the shoulder and elbow, by the University of California at Los Angeles and the Mayo Elbow Performance Score. Results: The incidence of iatrogenic radial nerve palsy was 5%. There was no failure of internal fixation Received: October 31, 2010 Accepted: May 22, 2011 Published online: October 19, 2011 Prof. Qiugen Wang Department of Orthopedic Surgery, Shanghai First People’s Hospital Medical College, Shanghai Jiaotong University 100 Haining Road, Shanghai 200080 (China) Tel. +86 138 0186 2558, E-Mail wangqiugen @ 126.com © 2011 S. متن کاملA Pitfall in Fixation of Distal Humeral Fractures with Pre-Contoured Locking Compression Plate
Anatomically precontoured locking plates are intended to facilitate the fixation of articular fractures and particularly those associated with osteoporosis. Fractures of the distal humerus are relatively uncommon injuries where operative intervention can be exceptionally challenging. The distal humeral trochlea provides a very narrow anatomical window through which to pass a fixed-angle locking...
متن کاملTips and Techniques- Surgical Fixation of Extra-articular Distal Humerus Fractures with a Posterolateral Locking Compression Plate (LCP)
The appropriate treatment of extraarticular distal humerus fractures is controversial. While functional bracing has been shown to result in good outcomes, some surgeons continue to favor operative fixation. Specifically, they cite concerns of radial nerve injury, difficulty in controlling fracture alignment, and long-term elbow stiffness with bracing. Jawa et al compared functional bracing with...
متن کاملRetrospective analysis of extra-articular distal humerus shaft fractures treated with the use of pre-contoured lateral column metaphyseal LCP by triceps-sparing posterolateral approach
Management of extra-articular distal humerus fractures presents a challenge to the treating surgeon due to the complex anatomy of the distal part of the humerus and complicated fracture morphology. Although surgical treatment has shown to provide a more stable reduction and alignment and predictable return to function, it has been associated with complications like iatrogenic radial nerve palsy...
متن کاملUnion Following Biological and Rigid Fixations of Distal Tibia Extra-articular Fractures
Background: Distal tibia fractures are among the most common bony injuries, with a significant rate of nonunion anddelayed union. There are multiple methods for the management of distal tibia fractures. Among the plating methods, there are bridge plating and compression plating techniques. There is still a lack of evidence about whether one methodhas a higher rate of union than...
